Healthcare Professionals stop burning out and save time with AI Phone Agent for Healthcare

For a typical UK clinic, AI phone cover starts at around £98 per month ex-VAT on a credit-based plan (that is the Motics Team plan, which includes the Phone agent alongside the Scribe and Chat agents in one shared credit pool). Flat per-agent pricing elsewhere in the market commonly runs at £100 to £150 per month for each phone line or agent, before VAT and before any overage. Human alternatives, such as outsourced virtual receptionist services, generally cost more per call handled because you are paying for a person's time on every call, and the bill scales in step with volume.

What you will actually pay depends on three things: your call volume, the vendor's pricing model (per agent, per seat, or credit-based), and what else is bundled in. This guide works through all three with real numbers and the arithmetic shown, so you can budget properly rather than guess. If you are still working out what these systems do in the first place, start with our 2026 guide to AI phone agents for UK clinics and come back here for the money.

The quick answer by clinic size

Clinic profile

Typical monthly cost (ex-VAT)

What drives the number

Small clinic, out-of-hours cover only (~40 calls/month)

From £98 credit-based, with most of the credit pool left over for clinical notes; £100-£150 per line on flat per-agent plans

~120 call minutes a month

Busier single-site clinic (~85 calls/month)

From £98 plus a small top-up (around £10), or a larger included pool as clinician count rises

~255 call minutes a month

Multi-site clinic with two phone lines

One shared credit pool on credit-based pricing; roughly double the fee on per-agent pricing

Per-agent multipliers versus shared usage

Larger clinic wanting note audits and usage analytics on top

From £290 credit-based

Higher volume plus audit and analytics features

Human virtual receptionist service (for comparison)

Varies by provider; typically priced per call handled, and usually the most expensive option per call at clinic volumes

A person answers every call

What are the three pricing models for AI receptionists?

Almost every AI receptionist on the UK market prices one of three ways. The model matters more than the headline number, because it determines how your bill behaves as your clinic changes.

Pricing model

How it charges

When it wins

Watch out for

Per-agent or per-line flat fee

A fixed monthly fee for each AI agent or phone line, commonly £100-£150/month in this market

Very high, steady call volume concentrated on a single line

Two lines or two sites means two fees; you pay the same in a quiet month as a busy one

Per-seat

A fee for each named user with access

Small teams where only one or two people need the system

Cost climbs with headcount rather than with calls; reception cover has little to do with how many logins you have

Credit-based credits

A shared pool of credits, drawn down by the minutes the agent actually spends on calls

Mixed or moderate volume, multi-site clinics, and clinics that also want AI notes or chat from the same pool

Check the overage rate and how the pool grows as the clinic grows

Two comparisons are worth making explicit.

High volume favours flat fees. If your line takes hundreds of calls a month and that volume is stable, a flat per-line fee caps your cost, and a credit-based plan needs a pool sized to match. Run the numbers both ways before assuming either is cheaper.

Multiple lines punish per-agent pricing. A clinic with two sites and two numbers pays twice on a per-agent model: two agents, double the fee, even if the second site takes a third of the calls. A shared credit pool does not care which line the minutes come from. This is one of the less obvious reasons credit-based pricing tends to suit growing and multi-site practices; we cover the scaling pattern in more detail in our piece on how mid-market UK practices scale with a phone agent.

How does credit-based pricing work?

Motics prices in credits, and the conversion is simple: 1 credit is roughly one Scribe note or ten chats, and phone calls cost 3 credits per minute.

The parts that matter for budgeting:

  • The pool is shared across the clinic. On the Team plan and above, every user draws from one clinic-wide pool. Users themselves are unlimited on Team and above, which is why the on-page line is "this sizes your credits, not your seats".

  • The pool scales with clinic size. Included credits are set by the number of full-time clinicians, not by lines or logins. Team starts at 704 shared credits a month at 2 clinicians and grows from there.

  • Unused credits roll over, up to 10% of your pool, so a quiet month is not entirely lost.

  • Overage is pay-as-you-go, not a penalty rate. PAYG applies only once the plan pool runs out: £0.19 per credit on Starter, £0.16 on Team, £0.13 on Scale.

  • Annual billing saves 15%, taking Team from £98 to £83 a month.

To turn credits into phone minutes, divide by three. The Team pool of 704 credits, spent entirely on calls, buys 234 minutes a month (704 ÷ 3 = 234.6). At £98 a month that works out at roughly £0.42 per minute of answered calls, before you count the notes and chats the same pool also covers. Per clinician per day, Team works out at roughly 16 Scribe notes, 160 chats or 5 phone minutes.

What would it cost your clinic? Two worked examples

The formula throughout: calls × average minutes per call × 3 credits per minute = credits used.

Example 1: small clinic, out-of-hours cover only

A two-clinician physiotherapy practice keeps its human reception in hours and switches the AI receptionist on for evenings and weekends. It takes about 40 out-of-hours calls a month, averaging 3 minutes each.

  • 40 calls × 3 minutes = 120 minutes

  • 120 minutes × 3 credits = 360 credits a month

That fits inside the Team plan's 704 shared credits with 344 credits to spare, which is roughly 344 Scribe notes' worth of clinical documentation from the same pool. Total cost: £98 a month ex-VAT (£83 on annual billing), with no overage.

Example 2: busier clinic, more of the day covered

A practice routes overflow and out-of-hours calls to the agent and takes about 85 AI-answered calls a month at the same 3-minute average.

  • 85 calls × 3 minutes = 255 minutes

  • 255 minutes × 3 credits = 765 credits a month

That is 61 credits over the base Team pool. Two ways to handle it:

  1. Pay-as-you-go top-up: 61 credits × £0.16 = £9.76, for a total of about £108 that month.

  2. Let the pool grow with the clinic: included credits scale with the number of full-time clinicians, so a three- or four-clinician practice has a larger pool at a correspondingly adjusted plan price, and the overage disappears.

Either way, the busier clinic is paying on the order of £110 a month ex-VAT for its call handling, not multiples of it.

When does an AI receptionist pay for itself?

This is the arithmetic that decides most purchases, so here it is plainly.

A private initial assessment in UK allied health commonly sits between £60 and £90. The break-even question is: how many bookings must the agent capture, that would otherwise have gone to voicemail, to cover its own cost?

  • Plan cost ÷ value of one recovered booking = bookings needed

  • £98 ÷ £60 = 1.6

So roughly two extra bookings a month covers the base plan, at the bottom of the fee range. At £90 per assessment, two recovered bookings bring in £180 against a £98 cost. Everything beyond that, including the value of returning patients behind each initial assessment and the reception time no longer spent returning voicemails, is margin.

Two bookings a month is a low bar. Our analysis of the hidden cost of missed calls in UK private practice found that roughly one in three calls to private practices goes unanswered, and out-of-hours callers rarely leave a voicemail and wait; many simply ring the next clinic on the list. If your clinic misses even a handful of would-be new patients a month, the agent does not need to be heroic to pay for itself. It needs to answer the phone.

What is bundled in the price, and what is extra?

Bundling changes the comparison more than most buyers expect, because many clinics shopping for a phone agent are already paying for adjacent tools.

With Motics, the Team plan's credit pool covers three agents, not one:

  • Phone agent: 24/7 or out-of-hours call answering, bookings, changes, cancellations, enquiries and callback requests, with every call logged with a summary, structured caller details and sentiment.

  • Scribe agent: recorded consultations (with patient consent) turned into structured notes, reports and letters from customisable plain-English templates.

  • Chat agent: a secure, compliant chat interface for using AI with real patient data.

The practical consequence: a clinic currently paying separately for a standalone AI scribe subscription should count that saving in the comparison. If the shared pool absorbs your documentation workload as well as your calls, the effective cost of the phone cover is the plan price minus whatever the scribe subscription was costing. For many small practices that materially changes the answer.

When comparing vendors, ask what the quoted price actually includes: some prices cover call answering only, with transcription, integrations, extra numbers or reporting sold separately.

The Phone agent arrives at Team level, so £98 a month ex-VAT is the entry price for AI call handling with Motics, and that price includes the Scribe and Chat agents from the same pool.

What hidden costs should you check with any vendor?

Whatever product you shortlist, put these questions in writing before you sign. None of them appear on pricing pages as prominently as the headline number.

  1. VAT. Almost all B2B software pricing in the UK is quoted ex-VAT. Add 20% to every figure when you budget, unless your clinic can recover it.

  2. Per-agent multipliers. If you have, or plan, a second line or a second location, ask exactly what doubles. On per-agent pricing the answer is usually the whole fee.

  3. Minimum terms. Some vendors quote a low monthly figure that is only available on a 12-month commitment. Check what the rolling-monthly price is, and what leaving costs.

  4. Overage rates. A cheap plan with an expensive overage rate can cost more than a dearer plan in any month your phone is busy. Ask for the per-minute or per-credit overage figure in writing and do the arithmetic at your real call volume.

  5. Onboarding and setup fees. Ask whether configuration, call-flow design or number porting carries a one-off charge. For reference, Motics setup pulls your clinic information from your website and Google Maps listing rather than starting from a blank form.

  6. What "unlimited" excludes. Unlimited users is not the same as unlimited minutes. Confirm which one the vendor means.

How to choose on cost: a short checklist

Price is only decidable against your own numbers, so gather them first.

  1. Count your calls. A month of call logs from your phone provider tells you total calls, out-of-hours calls and average duration. This is the single most useful input.

  2. Convert to the vendor's units. For usage pricing: calls × average minutes × the per-minute rate (in credits or pence). For per-agent pricing: fee × number of lines.

  3. Model a quiet month and a busy month. Flat fees are constant; usage bills move. You want to know both ends of your range, plus the overage rate that applies at the top end.

  4. Subtract what you already pay for. A bundled scribe or chat tool you would otherwise buy separately is a real saving; count it.

  5. Sanity-check against break-even. If two recovered bookings a month covers the plan, the precise pennies matter less than whether the agent actually answers well. Trial it on your own call patterns.

  6. Check the compliance floor before the price. A cheap agent that mishandles patient data is the most expensive option on the list. For UK clinics that means UK GDPR compliance as a minimum; for its part, Motics is UK GDPR compliant, Cyber Essentials certified, registered as a Class 1 medical device with the MHRA, deletes call audio within 48 hours and never uses customer data to train models.

Frequently asked questions

Is an AI receptionist cheaper than a human receptionist?

They do different jobs, so the honest answer has two parts. An AI receptionist does not replace your in-hours front desk team; it captures what they cannot get to: the calls that arrive while both staff are already on the phone, and everything outside opening hours. For that job, the comparison is stark: a salaried receptionist in the UK typically costs somewhere in the low-to-mid £20,000s a year before employer National Insurance and pension contributions, and no single hire covers evenings, weekends and simultaneous calls. AI cover from £98 a month ex-VAT is a small fraction of one salary and works all hours. If what you actually need is a person at the front desk during clinic hours, hire the person; the AI case is strongest for everything around them.

How many credits does a phone call use?

Three credits per minute on Motics. An average 3-minute call is 9 credits. Within the Team plan that is covered by the pool; beyond the pool it costs 9 × £0.16 = £1.44 per average call on pay-as-you-go.

What happens if we run out of credits mid-month?

Nothing stops working. Pay-as-you-go rates apply to usage beyond the pool (£0.16 per credit on Team), and unused credits in lighter months roll over up to 10% of the pool. If overage becomes routine, that is usually the signal that your pool should be sized to a higher clinician count or a higher tier.

Do the prices include VAT?

No. Like most UK B2B software, Motics prices are ex-VAT, and you should assume the same of any competitor quote unless it says otherwise. Budget the headline figure plus 20%.

Are there setup fees or minimum terms?

Ask every vendor this directly, because practices vary across the market and this is where surprise costs live. Motics offers monthly or annual billing (annual saves 15%) and a 30-day money-back guarantee; setup draws your clinic details from your website and Google Maps listing.

We have two sites and two phone numbers. Does the price double?

On per-agent or per-line pricing, usually yes: each line is a separate agent with a separate fee. On a shared credit pool it does not; minutes from both lines draw down the same clinic pool, and the pool is sized by how many clinicians you have rather than how many numbers you answer.

Is a credit-based plan risky if we get a busy month?

Only if you have not looked at the overage rate. On Motics Team, an unexpectedly busy month costs £0.48 per extra minute (3 credits × £0.16), so even a hundred extra minutes is £48, once, in the month it happens. Compare that with paying a higher flat fee every month to insure against it, and check any vendor's overage figure the same way.
